Confirmed bookings and the day-before reminder include a link to a two-minute form: why they are coming, health conditions, allergies, medicines and a typed signature.
Answers go straight into the patient's file. Allergies and conditions are added to their record, and the team is alerted to anything risky, such as a blood thinner or an anaesthetic allergy.
TipPatients who can't use the link can fill in the form at the clinic as before.
